Registration Fee: $10 (this fee may be waived if the child’s financial situation requires). The registration fee includes lunch on June 7 and June 8, dinner on June 8, snacks and beverages, as well as all supplies.

Deadline: May 23, 2008 (registrations received after this date will be on a space available basis only)

Registrations will be COMPLETE when the following have been submitted:

• online registration form
• parental permission form
• medical information form
• medication form
• $10 registration fee (or approved waiver of fee)

Registration fees can be made via check (make checks payable to Visiting Nurse Services of Michigan – Lansing).

Registrations will be confirmed by Visiting Nurse Services of Michigan – Lansing within one week of receipt.
